Forms are available at reception or online here.

Please complete forms and return with two forms of identification (one must show address and one must be photo id). An appointment with the Practice Nurse for a new patient screening can be arranged if required. 

The Practice does not discriminate on the grounds of race, gender, social class, religion, sexual orientation, appearance, disability or medical condition.

Patients will be registered with the Practice and not with a specific doctor.

Please ensure you have at least 4 weeks supply of any regular medication before registering as it can take a few weeks for the Practice to receive your medical records. We may not be able to provide prescriptions until we have these.

The Practice Area

The Practice area covers High Valleyfield, Low Valleyfield, Culross, Newmills, Torryburn, Cairneyhill, and Crossford. Please note we will not be taking on any new patients from Cairneyhill and Crossford. We can only accept new patients living in High Valleyfield, Newmills, Torryburn and Culross.
