Rights & Responsibilities

You have the following rights:-

    • That your medical records are confidential and an accurate up-to-date record of the care you receive

    • To receive the highest quality of care that we can provide

    • That we listen to you when discussing your medical care

    • To know how we use, share and protect your personal health information

    • To either speak to or see a doctor within 48 hours

    • To have access to 24 hour medical advice

    • To express a preference of practitioner and have this recorded in your notes if you wish

    • To be treated with dignity and respect and not discriminated against in any manner

    • To have access to information regarding healthy lifestyle and prevention of disease, illness and injury

    • To receive notification of any changes in working practices or policies (via posters, newsletter and website)

You have the following responsibilities:-

    • To arrive on time for appointments

    • To cancel any appointments with as much notice as possible

    • To ensure that you order repeat medication using our prescription policy and to order before you run out of any medication

    • To inform surgery of any changes to personal details

    • To only request a home visit when it is absolutely vital

    • To ask us to explain further if you do not fully understand any aspect of your medical condition or treatment

    • To treat all members of staff in a courteous manner. Abusive or aggressive behaviour towards staff may result in you being removed from our practice list
